Compounded versus FDA-approved GLP-1 medicines

Sharing an ingredient name does not make a compounded preparation an approved generic or establish that two products are interchangeable.

OUR TAKE

Sharing an ingredient name does not make a compounded preparation an approved generic or establish that two products are interchangeable.

What approval tells you

FDA-approved finished medicines have been reviewed for safety, effectiveness and quality for their approved uses. Their prescribing information describes the product, evidence, warnings and instructions. Wegovy and Zepbound have their own labels and formulation-specific information.[source][source]

That does not mean an approved medicine is appropriate for everyone. Eligibility, other medicines, medical history, preferences and the practical ability to follow the treatment plan still require clinician review.

What compounding changes

Compounded drugs are not FDA-approved. FDA does not review them for safety, effectiveness and quality before they are marketed. The agency says compounded GLP-1 medicines should be used only when a patient’s medical needs cannot be met by an approved drug.[source]

The phrase “same active ingredient” is therefore not a complete comparison. It does not prove the finished preparation has the same quality controls, clinical evidence or interchangeability as the branded medicine. Price alone cannot supply the medical rationale for compounding.

Which claims require extra care?

Do not transfer a branded study’s weight-loss percentage to a compounded program without evidence for that exact preparation and context. A provider’s patient story also does not establish the result a new patient should expect.

FDA has reported concerns involving dosing errors, products shipped with inadequate refrigeration, and salt forms such as semaglutide sodium or acetate. The agency distinguishes these from the active ingredient used in approved medicines and warns about unapproved online products.[source]

Questions for the prescriber and pharmacy

Ask why the proposed preparation is needed, which approved alternatives were considered, who will dispense it and how to contact that pharmacy. Confirm that the prescription, product label and instructions refer to the same preparation.

Resolve uncertainty about concentration, storage or measuring a dose directly with the prescriber or pharmacist. A syringe-unit instruction from another patient, social post or review is not transferable. This article deliberately does not provide a compounded dosing schedule.[source]

How our reviews preserve the distinction

CoreAge Rx and Mochi compounded-product reviews identify that status next to the price. Reviews of branded products through Ro, Hers or LillyDirect separately explain the care or fulfillment arrangement. The comparison does not treat them as identical products.

CoreAge Rx appears first because of commercial placement. That positioning is not FDA endorsement or evidence of clinical superiority. See the product reviews to understand the exact offering before comparing the full bill.
